The Bridges to Wellness program offers compassionate care through individualized nurse consultations and assessments, health education and activities, and care coordination that helps improve their clients’ overall health and well-being. This integrative model of care has been linked to reduced hospitalizations and emergency room visits, reduced risk of heart disease, and reduced homelessness. There is no other program in San Mateo County that addresses the physical health needs of people living with serious mental health conditions.
